Take your picture, you can download it with a little cost here just make it your self.

Then print out your pattern. Cut your print result to a smaller size in order to get easy process.

And then glue them to a heavy chipboard. Using a sharp scissors, cutting up all the shape. You can add the paper in the back of chipboard.

Then ink stain or distress the heart edges to hide the jagged cuts around it.

Make a small hole for the rivet before adding the matching color rivet.

The rest is simple, add your ribbon for the hanger just like the picture.
